Sustainable Neighborhood Network: Implementation Credit Form

Prior to beginning work on a credit-eligible initiative, participating neighborhoods should fill out the appropriate Proposal Form. After completing the initiative, the neighborhood must submit a Credit Form in order to receive credits towards certification.
